Part Time Social Worker/Case Manager

Job Description:

The HUF Center is currently looking to fill 10 part-time position in four different facilities (Richmond, Colonials Heights, Newport News, Norfolk, VA). The position is a part-time position (primarily on Saturdays between the hours of 7am-4pm). However, there will be opportunities to gain more hour during the week if you are available for PRN. These positions could transition into a full-time position based on the demand of the facility and individual work performance.

Outpatient Group Facilitator/Individual Counselor/Case Manager

Job Description:

 

• Responsible for providing ongoing case management services to assigned clients.
• Responsible for documenting services that each client participates in, as well as the level of participation on a daily basis.
• Responsible for facilitating psychosocial and psycho-educational specific groups to individual clients.
• Responsible for establishing an appropriate referral network with other agencies and service providers, to ensure comprehensive care.
• Responsible for daily documentation of observed demonstrated behavior as it relates specifically to the clients Individual Service Plan for all assigned clients.
• Responsible for attending weekly support team meetings with management.
• Responsible for completing  all discharge planning, monthly and annual reports as needed for all assigned clients.
• Responsible for recommending that clients be referred to other appropriate services or level of care.
• Responsible for completing ADL/IADL personal  care for clients specific to their individual needs.
• To perform other related duties as assigned by the Executive Director or Program Manager.

Must have graduated from an accredited college or university, and must possess a Bachelor's degree in Social Work, or a related field, with at least one years of documented experience with mentally impaired or mental retardation population; a current Cardiopulmonary Resuscitation (CPR)/ First Aid certification; trained in Handle With Care behavior management within thirty (30) days of employment, annual tuberculosis (TB) screening test; and receive satisfactory results from a criminal background check and a central registry check.

 

Must meet the Qualified mental health professional ( QMHP ) criteria. Qualified mental health professional (QMHP)" refers to a clinician in the human services field who is trained and experienced in providing psychiatric or mental health services to individuals who have a primary or secondary psychiatric diagnosis.

About Hope, Unity and Freedom Center:

The Hope, Unity and Freedom Center is a diverse team of professionals providing psychosocial day support to male and female adults with a primary mental health diagnosis, dual diagnosis, and/or a coexisting disorder; such as substance abuse.

The Hope Unity and Freedom Center facilitate services through small group activities that are conducted by Qualified Mental Health Professionals. Our staff utilizes a variety of therapeutic techniques and interventions that are designed to stabilize our client mental health, and improve their daily functioning skills. Individual therapy, group therapy, team building, behavior management, and crisis intervention are all services that are used in the program at the Hope Unity, and Freedom Center.

Our work helps to decrease psychiatric hospitalization and provide stabilization of individuals with less intervention in the community. By providing daily routine and structure in a safe environment each individual is able to achieve his/her best.

Responsible for on-site installing, maintaining and repairing company and multi-vendor systems which include hardware, software and networking products as well as operating systems. Ensures customer satisfaction by advising customers on preventative maintenance and configurations which may impact product performance. Takes responsibility for potential or desired follow-up services or problem escalation.

The General Duties:

· General hardware break/fix of laptops, desktops, some server work

· Troubleshooting and repair of hardware and software issues to maintain business continuity

· Diagnose, troubleshoot and repair Printers (HP, Lexmark), phones, peripheral equipment and other devices like Blackberries or handhelds

· New hardware upgrades; installs, moves, adds, changes in a laptop and desktop environment

· Optimizing Hardware/Software/Networking products and configurations as scripted ensuring customer satisfaction.

Role Requirements:

· High School diploma or GED required

· 2 or more years of experience in a hands-on desk side support role required

· Must have valid driver's license with reliable vehicle and appropriate insurance (required)

· Proficiency in Windows operating systems, Microsoft Office software

· Proficient with PC hardware repair and can troubleshoot/resolve software and operating system issues.

· Excellent communication skills required.

· A+ Certification required
